• Physician-supervised weight-loss protocols using FDA-Approved therapies
• Hormone-optimization programs with diagnostic testing, interpretation, and individualized dosing of FDA-approved therapies
• Educational content on wellness, nutrition, and life-style

Information on our website is for general educational purposes only and does not constitute medical advice. Medical services are delivered only after establishing a patient-physician relationship through proper registration and informed consent.

Scope of Practice & Provider Responsibilities

All clinical services are provided by licensed Florida physicians and credentialed clinicians under physician supervision, who are solely responsible for the delivery of any medical services. Thrive Rx ensures providers meet state licensure requirements. Thrive Rx does not guarantee specific outcomes, and individual results vary.

Privacy

Your use of the Services with respect to privacy is governed by our Privacy Policy. Please review it to understand how your data is collected, used, and protected.

Account Registration and Use

To become a patient of the medical providers, you must complete the intake process, provide accurate personal and medical information, and agree to our informed-consent and membership documents. You are responsible for maintaining the confidentiality of your account credentials and for all activities under your account. You must be 18 years or older.

User Responsibilities

By accessing the Services, you agree to: Use the Services for personal, non-commercial purposes only; Not copy, distribute, or resell any content or functionality; Not upload or transmit content that infringes on third-party rights; Comply with all applicable laws and regulations; Not impersonate others or post offensive, defamatory, or illegal content; Protect your account credentials and notify us immediately of any unauthorized access; Not transfer your account to another individual; Not disrupt or interfere with the Services or network operations; Provide accurate, complete and updated information; Follow medical advice and safety instructions provided by your clinician. Violation of these terms may result in immediate suspension or termination of access.

User-Generated Content

You are solely responsible for any content you share on the platform. Thrive Rx LLC does not monitor user content but reserves the right to remove or modify content that violates our policies or is deemed inappropriate.

Fees and Payment

Fees for the personalized medical services are listed on our website and are subject to change. Users are responsible for staying informed about current rates. Thrive Rx LLC operates on a direct-pay (cash-based) model and does not bill insurance. It is your responsibility to verify coverage with your health plan. All fees must be paid upfront, regardless of potential reimbursement.

• Fees are due upfront on a monthly basis
• A three-month minimum commitment applies to all programs.
• If you pay in full and request cancellation, a pro-rated refund may be issued after the three-month minimum, less any completed services.

Intellectual Property

Thrive Rx LLC and its affiliates retain all rights, titles, and interests in the Services, including intellectual property rights. Any feedback or suggestions you provide become the property of Thrive Rx LLC. You may not: Sell, distribute, or commercially exploit the Services; Modify or create derivative works; Reverse-engineer or replicate the Services; Use automated tools to disrupt or overload the platform. You are granted a limited, non-exclusive, non-transferable license to use the Services for personal purposes only.

Disclaimer of Warranties

The use of this website is provided “as is” and “as available” and We makes no guarantees regarding results, outcomes, or uninterrupted access. Nothing on the site constitutes a promise or warranty of specific weight loss or hormone balance. This website does not provide medical advice, diagnosis, or treatment. We are not liable for any damages, including data loss or technical issues. If a court finds any limitation of liability unenforceable, Thrive Rx LLC’s maximum liability will be limited to $1,000.

Indemnification

You agree to indemnify and hold harmless Thrive Rx LLC and its affiliates from any claims, damages, or expenses arising from your use of the Services or violation of this Agreement.

Service Modifications and Termination

Thrive Rx LLC may modify or discontinue the Services at any time without notice. We are not liable for any resulting impact.

This Agreement is governed by the laws of Florida. Any disputes will be resolved through arbitration under the rules of the American Arbitration Association in Florida.

Emergency Situations

Thrive Rx LLC does not offer emergency medical services. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency facility immediately.

Communications

We may communicate with you via our secure, HIPAA-compliant EHR portal. Text messages or email may be used for administrative matters (e.g. reminders, scheduling). Medical communications will only occur through our encrypted platform. By opting into messaging, you consent to receive such communications, including promotional content and appointment reminders. Message and data rates may apply. You must be at least 18 years old and reside in the U.S. If your mobile number changes, please notify us to update your preferences.
